Everything on this site is pulled directly from the CMS (Centers for Medicare & Medicaid Services) Provider Data Catalog — the same source that powers Medicare's own Nursing Home Compare. We refresh our copy of the data on a rolling basis so citations and fines are current.

Most inspections happen on a regular schedule. Some are triggered by a specific complaint filed against the facility — those "complaint" surveys can happen any time and often indicate a more serious, resident-reported issue. We separate them from routine health inspections so you can see the difference at a glance.

CMS grades every citation on two axes: scope (how many residents were affected — an isolated issue vs. a widespread pattern) and severity (how serious it was — from minor potential for harm, all the way up to immediate jeopardy). The letter grades A–L are combinations of the two. Every citation on our site is rendered with a plain-English sentence instead of just the letter, e.g. "resulted in actual harm to one or more residents, affecting a limited number of residents, staff, or situations."
