1Nearby search
- Pick a radius from the dropdown (5, 20, 50, 100, or 500 miles).
- Click Nearby. Your browser will ask for location permission the first time.
- Only states, cities, and facilities within that radius are shown. The selected state auto-advances to the nearest one.
- Click Clear location at the top-right of that bar to remove the radius filter and return to the full directory.
2States
- Click one of the state badges in the State row. The small number on the badge shows how many facilities are in that state.
- Click the same badge again to deselect it and clear the city row.
3Cities
- The blue badge next to City shows the number of cities currently shown after filters are applied.
- Type in the Filter cities... box to filter the badge row by name.
- Click the currently active city badge again to deselect it.
4Filters
Every filter works the same way. Pick a value to require it. Click Any to clear that specific filter. Filters stack, and the results must match all of them.
- Fines, Health Deficiencies, and Health Complaints:
Any, Yes, or No. Yes shows only facilities that have at least one of that item on record. No shows only those with none.
- Overall, Health, and Staffing rating:
Any or a specific star value from 1 to 5. Requires that exact rating.
- Ownership: one-click badges for the ownership types present in the current filtered set. Click a badge to require that ownership. Click the same badge again to clear.
Filters stay set when you switch states, so you can compare the same criteria across several states back-to-back.
Resetting filters
- Clear all (top-right of the Filters section) resets every filter back to
Any. Your State and City selections stay put.
- Clear location (top-right of the radius bar, only visible when Nearby is active) removes the radius filter and also resets all filters.
- Click the active State badge again to deselect the state and clear the city row.
5Reading a facility's record
Click any facility in the list on the left. Its full record opens on the right, and the page scrolls down to it. The record is organized into tabs:
- Details: always free. Address, phone, county, ownership, certified beds, health inspection rating, and staffing rating.
- Fines: every penalty on record, with the amount and the date. Each fine shows the deficiencies that happened on the same date as a bar chart.
- Health: the routine health inspection citations, grouped by deficiency category.
- Health Complaints: citations that were triggered by a specific complaint.
Inside the Fines, Health, and Health Complaints tabs, each bar is a category. The bar colors are severity tiers, from least to most serious:
Minor
Potential for more than minimal harm
Actual harm
Immediate jeopardy
Click any colored bar or bar segment to see a plain-English explanation of that severity rating and the exact dates it occurred on and how often.
6Unlocking Fines, Health, and Health Complaints
The Fines, Health, and Health Complaints tabs are locked by default. A small padlock icon on the tab tells you it is not unlocked yet.
- Click the locked tab. A confirmation dialog explains the cost: 1 credit.
- Click OK. The section unlocks and the tab's padlock icon changes to an open lock.
- Once unlocked, that section is saved to your account forever. You can come back to it anytime, on any device you sign into. You will never be charged again for that section on that facility.
Each unlock is per facility, per section. Unlocking Fines for one facility does not unlock Health for that same facility, nor any section for a different facility.
7Credits and the Upgrade button
- New accounts start with 6 free credits. No card required. Each credit unlocks one section on one facility.
- The badge next to the account row shows your current credit balance (visible when signed in).
- Click Upgrade to buy more credits. Pick the exact credit pack you want with the plus and minus stepper, then continue to PayPal checkout.
- If you are not signed in yet, clicking Upgrade (or trying to unlock a section) opens the sign-in dialog. Enter your email and we will send you a magic sign-in link. No password needed.
- Purchased credits never expire.
8My Unlocked Facilities
Click My Unlocked Facilities in the account row to see every facility where you have unlocked at least one section. Each card lists which sections you have unlocked. This is a fast way to compare a shortlist side by side. Click any card to reopen its full record.
9Printing and saving a record
- Open a facility's record.
- Click Print at the top-right of the Facility Detail panel.
- The browser's print preview opens. Choose Save as PDF or a printer.
- Layout: the Details block and the first available content section print on page 1 together. Every additional section starts on its own page, and each fine card also starts on its own page so the charts stay readable.
